I have used Calibre for many years, but recently have been having random problems with covers reverting to other books, or the wrong book file appearing in an already added book. Finally decided to get serious about tracing the issue and found that when I use the "Open Containing Folder" option for a book it is opening the root Calibre library foler, instead of 2 levels down in Author-->Title. I also found that my most recent book files (10-15) were also in this folder, as well as a single cover.jpg file. This explains why my system has been having trouble with both books and covers when creating/modifying them.
