The TearCare ® System (Sight Sciences, USA) is an electrothermal controller that heats both eyelids to 41–45 °C by adjusting them with a flexible, single-use material called iLid™. The system encourages meibum expression through normal blinking during the treatment and through the subsequent manual MG expression—the set includes a disposable EXPRESS™ Forceps to perform the manoeuvre. Badawi et al. [ 22, 23] performed a clinical trial (I) evaluating the effectiveness of the TearCare ® System and the MGDRx EyeBag ® in two different groups of MGD patients. While one group underwent the TearCare ® for 12 min and subsequent MG expression, the other group self-applied 5 min of daily therapy with EyeBag ® for 4 weeks. The TearCare ® group showed a higher improvement in signs, including MG scores, and symptoms. Additionally, the effects lasted for 6 months, and no adverse events were reported. If you have a plate warming setting on your oven, use that. If not, set your oven to the lowest temperature which should be 50 to 60 degrees C.

Prior to using this product, I was using the warm wet compresses twice a day. The problem with that is that the skin around my sensitive eyes were getting extremely dry and painful every time I applied the wet compresses (because they were so dried out from doing this for so long). I've have now been using the MGD Eyebag for two weeks now and so far so good. I don't see any symptoms of the blepharitis I had been experiencing before (swollen/puffy bottom lids, crusting around both ends of the eyes). The instruction booklet is very detailed on how to use it and how to warm it up and how long it last for.

The Blepha EyeBag (Formally MDG Rx Eyebag ®) is a patented, CE marked Class 1 medical device registered with the Medicines and Healthcare products Regulatory Agency (MHRA). Eyelid temperatures, NITBUT, and TFLLT increased significantly from baseline in test eyes immediately after removal of the heated eyebag compared with those in control eyes (maximum temperature change, 2.3 ± 1.2 °C vs. 0.3 ± 0.5 °C, F = 20.533, p < 0.001; NITBUT change, 4.0 ± 2.3 seconds vs. 0.4 ± 1.7 seconds, p < 0.001; TFLLT change, 2.0 ± 0.9 grades vs. 0.1 ± 0.4 grades, Z = -4.035, p < 0.001). After 10 minutes, measurements remained significantly higher than those in controls (maximum temperature change, 1.0 ± 0.7 °C vs. 0.1 ± 0.3 °C, F = 14.247, p < 0.001; NITBUT change, 3.6 ± 2.1 seconds vs. 0.1 ± 1.9 seconds, p < 0.001; TFLLT change, 1.5 ± 0.9 vs. 0.2 ± 0.5 grades, Z = -3.835, p < 0.001). No adverse events occurred during the study. These devices apply heat to the eyelids to liquefy the meibum and then apply a mechanical pressure to deliver the meibum to the ocular surface.

If this is the only way of heating your EyeBag, ensure that you place it in a clean closed ovenproof container with a well fitting lid. Visual acuity: best corrected monocular and binocular visual acuity was measured using a digital logMAR chart at 6 m, randomised between presentations (Test Chart 2000; Thomson Software Solutions, London, UK). Visual acuity was determined using letter by letter scoring with each letter corresponding to −0.02 logMAR units.

The eyeXpress™ Eye Hydration System (Holbar Medical Products, USA) is a goggle system, which provides constant but adjustable heat to the anterior surface of both eyelids. Two studies (III) suggested that Blephasteam ® might be a safe and effective therapy for MGD because DED symptoms and MG function improved after 21 days of treatment [ 30, 31]. Ocular symptoms and clinical signs, except the conjunctival redness score, may achieve a higher improvement after Blephasteam ® than after warm compresses (III) [ 32]. However, in another study (I), signs improved equally in both therapies [ 33]. Nonetheless, Blephasteam ® may be useful for subjects who find warm compresses ineffective (III) [ 34]. Warming and Massaging Devices The LipiFlow ® system (Johnson & Johnson Vision, USA) applies heat (42.5 °C) and simultaneous pulsated pressure to the inner upper and lower eyelids for 12 min. The device consists of an eyelid warmer that applies the heat and an eye cup that applies the pressure through an inflatable air bladder [ 35]. The Blephasteam ® (Théa Laboratories, France) is a goggle system that produces moist warm air and heats the eyelids through the condensation of water vapor. The melted meibum secretion is favoured by the normal blinking of the patients during the treatment. Temperatures recorded in the outer upper eyelids were between 37.4 °C and 40.0 °C at the end of the treatment, while in the lower ones they were between 37.8 °C and 38.4 °C [ 27, 29].Doan S, Chiambaretta F, Baudouin C.
